FAQ's about PIAA physicals:

When are the dates for the 2017-2018 physicals?

Physicals for the fall season must be completed AFTER June 1st 2017. and no later then the start of the season. Physicals dated before June 1st are not valid to satisfy the PIAA physical guidelines. All forms dropped off to the main office should be clearly labeled PIAA form with the student athlete's name along with Attention-Sylvia Doran. Parents should enclose a copy of any plan of care forms for medical conditions such as asthma or diabetes.
My insurance will not pay for another physical in this calendar year.
Speak to your family physician about this. They might offer ways to help offset the cost. CVS and Walgreen's offer sports physicals at a reasonable cost.
Who needs a PIAA sports physical?
EVERY student-athlete trying out for a team. Every student-athlete will have a new emergency contact form on file each sports season.

What about the 11th grade school physical, may I use that?
No. The PIAA sports physical is an inclusive evaluation of the student-athletes health in regard to vigorous physical activity. The nurse will take the PIAA physical copy to satisfy the 11th grade physical requirement, however, we must have the PIAA physical completed for athletics.
When are these forms due?
Football is due August 7, 2017, all other fall sports are due August 10, 2017. There will be no exceptions if these forms are not complete and handed in to the athletic trainer or coach. Unfortunately, the student-athlete will not participate in any form on the first day.
Do I need these forms just to try out for a team?
